It is built on a GIP backbone rather than a GLP-1 one, which is the structural feature behind its dual-agonist profile and the reason it is studied alongside, rather than as a substitute for, single-agonist GLP-1 analogues. Alpha-aminoisobutyric acid (Aib) at positions 2 and 13 resists dipeptidyl peptidase-4 cleavage, and an eicosanedioic (C20) fatty diacid attached to the Lys20 side chain through a gamma-glutamyl and two AEEA spacers binds serum albumin, extending circulating half-life in the models studied. Research has investigated the combined incretin signalling observed in preclinical models: glucose-dependent insulin secretion, insulin sensitivity in adipose and skeletal muscle tissue, lipid handling, and central appetite regulation.
