SS-31 (elamipretide, MTP-131) is a synthetic aromatic-cationic tetrapeptide — D-Arg-Dmt-Lys-Phe-NH2, identified by CAS number 736992-21-5 — designed to concentrate in the inner mitochondrial membrane. Its alternating aromatic and cationic residues make it cell-permeable without a transporter, and the D-arginine and 2,6-dimethyltyrosine substitutions resist peptidase degradation. Unlike compounds that depend on membrane potential to accumulate, SS-31 associates selectively with cardiolipin, the signature phospholipid of the inner mitochondrial membrane, and preclinical research has characterised the consequences of that association: stabilised cristae architecture, improved electron-transport-chain coupling, and reduced peroxidase activity of cardiolipin-bound cytochrome c. Research has examined these effects in ischaemia-reperfusion, heart-failure, skeletal-muscle-ageing, and renal-injury models.
