Semaglutide is a synthetic 31 amino acid analogue of human glucagon-like peptide-1 (GLP-1), identified by CAS number 910463-68-2. Two structural modifications distinguish it from native GLP-1 and are the basis of most research interest: alpha-aminoisobutyric acid (Aib) substituted at position 2, which resists cleavage by dipeptidyl peptidase-4, and an octadecanedioic (C18) fatty diacid attached to the Lys20 side chain through a gamma-glutamyl and two AEEA spacers, which binds serum albumin and markedly extends circulating half-life in the models studied. Research has characterised its agonism at the GLP-1 receptor and the downstream effects observed in preclinical models: glucose-dependent insulin secretion, suppression of glucagon release, delayed gastric emptying, and central appetite signalling in hypothalamic and hindbrain nuclei.
